First, remember to choose a date!  Then fill out the form below at least three weeks in advance of the Conference you plan to attend.  Please fill out all the data items.  If some are not applicable, enter an "NA".  If you supply an email address, an email confirmation will usually be sent within 48 hours.  If no email address is supplied, confirmation letters will be sent to the bride's address two weeks prior to the Conference you have chosen.  There is a $30 fee that will be collected at the door of Assembly Hall at St. Mary's Health Center on the day of your Conference.  If you have questions, email us at jcprecana@jcprecana.info.
